Vermont DOT Physicals with Dr. Windy Heyn

$140 — Typically 60 Minutes

Your DOT physical includes a full review of your health history, required screenings, and completion of DOT/FMCSA medical forms. If you qualify, you will receive your DOT medical card at the end of your visit.

What to Bring & Medical Clearances

Some drivers are required by the Federal Motor Carrier Safety Administration (FMCSA) to provide additional medical documentation before a DOT medical certificate can be issued. If any of the conditions below apply to you, please bring the appropriate clearance form completed by your treating provider to your appointment.

DOT Physical FAQs

Many drivers qualify for a 2-year DOT medical card. Some medical conditions—such as high blood pressure, diabetes, or sleep apnea—may require a shorter certification period so your health can be monitored more closely.
No. The DOT physical includes a required urine test, but it is used to screen for certain medical conditions, not drugs. If your employer requires drug and alcohol testing, that is a separate service and process.
